Clean validation patch for openai#37893. Adds optional InitializeResponse.processId, propagates it through daemon probe, and safely reconstructs app-server.pid when state is lost. Adoption requires expected CODEX_HOME, stable process start time, and executable identity; Unix also requires exact argv. start attempts safe adoption instead of treating a healthy socket as terminal. Protocol fixtures are updated.
