ref(node): Vendor kafkajs instrumentation

[nicohrubec]

Vendors @opentelemetry/instrumentation-kafkajs into the SDK with no logic changes. Types from kafkajs are inlined as simplified interfaces to avoid requiring the package as a dependency.
