same here, All chat prompts ends with same error message every time today:

the github-mcp-server works fine through Copilot CLI and vscode. Only GitHub Copilot App affected.

Removing mcp cache, restarting and re-authenticating doesn't help.

Hitting this exact same error on Copilot Desktop — Execution failed: Failed to refresh MCP tools for server github-mcp-server; the last catalog is stale. Confirms this isn't isolated to one setup. Starting a new chat clears it temporarily, but it recurs. Happy to share more repro details if useful.

Same issue here - v1.1.21/Windows 11 - no issues prior to installing this version. This is making the app more or less unusable for me - restarting fixes for a short while but the issue always comes back.
