Format validation currently executes agent actions, causing CodeAgent to run twice for one Worker action.
This change checks the action structure and function signature without executing the tool. It also prevents an invalid final response from being executed after formatting retries are exhausted.
Verified with 9 targeted regression checks using model/controller stubs.
Fixes #232
